缺氧性肺动脉高压新生儿右心室Tei指数与肺动脉压力及血气分析指标的相关性研究

目的 探讨缺氧性肺动脉高压(HPH)新生儿右心室Tei指数与肺动脉压力及血气分析指标的关系.方法 对80例HPH患儿(轻度30例、中度30例、重度20例)及对照组30例于生后1、3、7 d以超声心动图估测肺动脉收缩压(PASP),测右室Tei指数(RV-Tei),测定血氧分压(PaO_2)、PaO_2/FiO_2,FiO_2为吸氧浓度.结果 生后1 d HPH三组PaO_2、PaO2_/FiO_2降低,PASP及RV-Tei升高;生后3 d轻、中度组PaO_2、PaO_2/FiO_2恢复;生后7 d轻、中度组以上指标均恢复,重度组PASP及RV-Tei仍高.结论 HPH新生儿血气指标恢复快,肺动脉压力及右心室功能恢复慢,Tei指数可以敏感地发现其右心室功能改变.

Correlation of Tei Index of Right Ventricular and Pulmonary Arterial SystolicPressure as Well as the Indexes of Blood-gas in Neonates with Hypoxic Pulmonary Hypertension

Objective To evaluate the relation of Tei index of right ventricular and pulmonary arterial systolic pressure(PASP),as well as the indexes of blood-gas in neonates with hypoxic pulmonary hypertension (HPH).Methods Serial echocardiographic examinations were performed on day 1,day 3,and day 7 after birth on 110 cases of neonates,which were divided into 4 groups (30 cases of mild HPH,30 cases of moderately severe HPH,20 cases of severe HPH,and 30 cases of control).Evaluate pulmonary artery systolic pressure (PASP) with pulsed Doppler echocardiography,determine Tei index of right ventricular (RV-Tei),and at the same time take blood analysis for artery oxygen pressure (PaO_2),record concentration of oxygen (FiO_2),PaO_2/ FiO_2 value.Results 1) On day 1 after birth,in three group of HPH neonates,PaO_2,PaO2_/ FiO_2 were lower than those of control group,while PASP and RV-Tei were higher.On day 3 after birth,PaO_2,PaO_2/FiO_2 recovered in mild and moderate HPH group.On day 7 after birth,all indicators were resumed in mild and moderate HPH group,while PASP and RV-Tei were still high in severe HPH group.Conclusions The indexes of blood-gas of HPH newborn recovered rapidly,while pulmonary artery pressure and right ventricular function recovered slowly.Tei index can reflects changes of its right ventricular function sensitively.
