地塞米松联合大剂量丙种球蛋白治疗儿童急性重症血小板减少性紫癜的疗效观察

摘    要:目的:探讨地塞米松联合大剂量丙种球蛋白治疗儿童急性重症血小板减少性紫癜的疗效。方法:31例患儿分为对照组20例,治疗组11例。治疗组在对照组地塞米松治疗基础上予以静脉用大剂量丙种球蛋白(HDIVIG),治疗前及治疗后1周内每日复查血小板计数。结果:治疗组血小板计数在予HDIVIG后24h、48h、72h,血小板升高值明显高于对照组,临床出血症状改善更为显著,血小板在48h内达安全范围。结论:地塞米松联合大剂量丙种球蛋白治疗儿童急性重症血小板减少性紫癜,能短期内迅速提高血小板计数,降低重要脏器出血风险,值得临床推广应用。

Abstract:Objective:To investigate the efficacy of dexamethasone and high dose immunoglobulin (HDIVIG) combination treatment in childhood acute severe idopathetic thromcytopenic purpura(ITP). Methods:31 children patients with severe acute ITP were enrolled in this study. Two groups were set up,of which the control group (n=20) were treated with dexamethusone,the treatment group(n=11) were treated with dexomethasone and HDIVIG, complete blood count was tested each day during the first 7 days period. Results:The platelet counts were significantly higher during the first 3 days in the treatment group than in the control group, and platelet counts was elevated to a safe range in 48 hours, and no hemorrhage in vital organs occurred in the treatment group. Conclusion: Dexamethasone and HDIVIG combination therapy has better results in childhood acute severe ITP, it can raise platelet count to safe ranges in shorter periods and thus reduce the risk of fatal hemorrhage.
