P16和cyclinD1在鼻咽癌及癌前病变中表达的临床意义

摘    要:目的研究鼻咽癌及癌前病变组织中P16和cyclin D1蛋白的表达、相互关系及其意义。方法采用免疫组化EnVision法,分别检测60例鼻咽癌、28例癌前病变及30例鼻咽粘膜慢性炎组织中P16和cyclin D1蛋白的表达情况,分析两者在不同病变组织中的表达以及与鼻咽癌临床病理特征的关系。结果P16蛋白在鼻咽粘膜慢性炎组织的表达率高达90%(27/30),并随着病变进展,从癌前病变(不典型增生)至鼻咽癌,其阳性表达率呈下降趋势分别为64.3%(18/28)和28.3%(17/60);其中癌前病变和鼻咽癌与鼻咽粘膜慢性炎组织比较有统计学意义(P〈0.05),并且P16蛋白的低表达与临床分期和淋巴结转移有关。eyelinD1蛋白在鼻咽粘膜慢性炎、癌前病变和鼻咽癌组织的阳性表达率分别为10%(3/30)、57.1%(16/28)和63.3%(38/60)。其中鼻咽癌与鼻咽粘膜慢性炎组织比较有统计学意义(P〈0.05),而鼻咽癌与癌前病变比较则无统计学意义(P〉0.05),eyelinD1蛋白的过表达与临床分期和淋巴结转移无关。P16、eyelinD1蛋白的阳性表达率在鼻咽癌组织中呈负相关性(P〈0.05)。结论P16、eyclinD1在鼻咽粘膜癌变过程中起着重要作用,P16基因的畸变或缺失可能参与鼻咽癌的发生发展过程;eyelinD1的过表达是鼻咽癌发生的早期事件。

Clinical significance of P16 and cyclinD1 expression in nasopharyngeal carcinoma and precancerous lesion

Abstract:Objective To detect the expression of PI6 and cyclinD1 protein in nasopharyngeal carcinoma and precancerous lesion, and elucidate the relationship between them. Mehtods The expression of P16 and cyclinD1 protein expression in 60 cases of nasopharyngeal carcinoma, 28 cases of precancerous lesion and 30 cases of inflam-matory nasopharyngeal mucosa were detected by immunohistochemistry method. Results The expression rate of P16 protein in inflammatory nasopharyngeai mucosa was 90% (27/30), 64.3% in precancerous lesion and 28.3% in nasopharyngeal carcinoma, which was significantly different from that in inflammatory nasopharyngeal mucosa by statistic analysis (P<0.05). And the expression rate of P 16 protein was correlated with clinical stage and lymph node metastasis. The expression rate of cyclinD1 protein was 10% (3/30) in inflammatory nasopharyngeal mucosa, 57.1% (16/28) in precancerous lesion and 63.3% (38/60) in nasopharyngeal carcinoma, respectively. The expression rate of cyclinD1 protein in nasopharyngeal carcinoma was significantly different from that in inflammatory nasopharyngeal mucosa (P<0.05), and was not significantly different from that in precancerous lesion (P>0.05). And the expression rate of PI6 protein was not related with clinical stage and lymph node metastasis. In addition, the expression rate of cyclinD1 protein was negatively correlated with that of P16 in nasopharyngeal carcinoma. Conclusions P16 and cyclinD1 play an important role in nasopharyngeal carcinogenesis by mutation or loss of PI6 gene. And the over expression of cyclinD1 protein may be an early thing during nasopharyngeal carcinogenesis.
