改良封闭负压引流术在口腔颌面部感染中的临床应用

目的 探讨利用改良封闭负压引流装置合并脓肿切开引流术治疗口腔颌面部感染的临床应用效果.方法 收集80例口腔颌面部感染患者的资料,采用病例对照研究方法,40例采用改良封闭负压引流技术治疗（实验组）,40例采用传统的切开引流术治疗（对照组）.结果 对照组1例口底蜂窝织炎患者因纵膈感染死亡,实验组中1例口底蜂窝织炎患者因多器官功能衰竭死亡,其余均治愈,治愈率均为97.5％.与对照组相比,实验组伤口愈合时间及抗生素使用时间短,手术切口小,差异有统计学意义（P＜0.05）.结论 改良封闭负压引流技术治疗颌面部感染取得了较好的临床效果,使患者痛苦小,创面愈合快、更换敷料次数少、抗生素使用时间缩短,并且具有美观、安全、手术创伤小、可保护重要组织的效果.

Clinical application of improved vacuum sealing drainage in oral and maxillofacial infections

Objective To evaluate the clinical efficacy of improved vacuum sealing drainage combined with abscess incision drainage treatment of oral and maxillofacial infections.Methods Collection of 80 cases with oral and maxillofacial infections were investigated by case-control study.40 cases were treated with improved vacuum sealing drainage （experimental group）,another 40 cases were treated with traditional incision drainage（control group）.Results One of the mouth floor cellulitis patients in control group treated with conventional treatment methods died because of mediastinum infection.One case of cellulitis patients in experimental group treated with improved vacuum sealing drainage died,due to multiple organ failure.All the rest were cured.Both of the cure rate were 97.5％.Compared to the control group,the time of healing and antibiotic using in experimental group was relatively shorter,as well as a smaller incision.The difference was statistically significant （P ＜ 0.05） Conclusion Improved vacuum sealing drainage in the treatment of oral and maxillofacial infections has obtained good results,which makes little pain,faster healing,less times of dressing changes,reduce the use of antimicrobial agents,it has beauty,safety,small surgical trauma and the effect of protecting important organization.
