超声二维斑点追踪技术评价男性重度吸烟对左心室收缩功能的影响

摘    要:目的 应用超声二维斑点追踪技术评价男性重度吸烟对左心室收缩功能的影响.方法 获取82例重度吸烟男性与65例无吸烟史健康男性的标准心尖位左心流出道切面、四腔心切面、两腔心切面、短轴左心室二尖瓣水平、乳头肌水平、心尖水平的动态图像,应用超声二维斑点追踪技术测量左心室心尖系列切面18节段纵向应变及左室短轴系列切面径向、圆周18节段应变,并测量二尖瓣水平及心尖水平左心室的旋转角度,比较重度吸烟组与无吸烟史组的收缩期峰值应变和旋转角度.结果 与无吸烟史组相比,重度吸烟组左心室收缩期峰值纵向应变、峰值圆周应变、二尖瓣水平的峰值径向应变及旋转角度均低于无吸烟史组(P<0.05);短轴乳头肌水平、心尖水平的径向应变明显低于无吸烟史组(P<0.01).结论 超声二维斑点追踪技术可以准确地反映重度吸烟对男性人群左心室收缩功能的影响,对其心功能判断具有重要价值.

Abstract:Objective This study was performed to evaluate the left ventricular systolic function in men with heavy smoking using two-dimensional speckle tracking echocardiography.Methods Dynamic images were recorded from the left ventricular apical long-axis view,the apical four-chamber view,two-chamber view and the short-axis view at the mitral orifice,papillary muscle and apical levels in 82 men with heavy smoking and 65 healthy men without smoking.The peak systolic longitudinal strains of 18 segments were measured in apical views,and the peak systolic circumferential strain,radial strain of 18 segments were measured in the left ventricular short-axis views using two-dimensional speckle.Rotation angles of the mitral orifice and apical levels of left ventricle were measured.The peak systolic strains and rotation angles were compared between heavy smoking men and healthy men without smoking.Results When compared with healthy men without smoking,left ventricular peak longitudinal strain,peak circumferential strain,peak radial strain at the mitral orifice level,and rotation angle in men with heavy smoking were lower than those men without smoking(P < 0.05).Peak radial strain at the papillary muscle and apical levels in men with heavy smoking were lower obviously than those men without smoking (P < 0.01).Conclusion Two-dimensional speckle can accurately reflect the impact of heavy smoking of male population on left ventricular systolic function and has important value to determine heart function.
