老年尿路感染者病原菌及其耐药性分析

摘    要:目的研究老年泌尿系感染临床分离细菌分布及主要病原菌的耐药性.方法采用Microscan autoscan-4细菌药敏分析仪鉴定进行药敏试验.结果我院泌尿系感染的细菌检出率为27.70%.主要病原菌为大肠埃希氏菌和G^+球菌.大肠埃希氏菌对亚胺培南、头孢哌酮-舒巴坦较敏感,对喹诺酮类、磺胺类及头孢呋辛酯耐药率高,对第三代头孢菌素有上升趋势.G^+球菌普遍耐药,唯对万古霉素敏感,但亦有耐药菌株出现.结论老年泌尿系感染的细菌以大肠埃希氏菌及G^+球菌为主,对常用抗生素耐药有上升趋势,应定期监测细菌的耐药趋势,指导临床用药.

Abstract:Abstract] Objective To investigate the distribution and antimicrobial resistance of pathogens in elder adult patients with urinary tract infection. Methods Microscan Autoscan-4 system was adopted to identify pathogens in 241 elder adult patients with urinary tract infection and to test the antimicrobial susceptibility. Resuits The positive rate of clearing urine cultivation was 27.70%. The most common pathogens were escherichia coli and gram-positive cocci. The antimicrobial resistance rate of escherichia coli to immipenam and cefoperazone-subactarn was low, to quinolones, suffonamides and cefuroxime axetil was high, and the antimicrobial resistance rate to third generation cephalosporin was rising. Gram-positive cocci was only susceptible to vancomycin, but there were gram-positive cocci strains resistant to vancomycin. Conclusions The most common pathogens were escherichia coli and gram-positive cocci in elder adult patients with urinary tract infection, and their antimicrobial resistance is rising. We should regularly survey the antimicrohial resistance to guid clinical using antibiotics rationally.
