尿微量蛋白联合检测对糖尿病早期肾损伤的诊断价值

摘    要:目的观察糖尿病患者尿液微量蛋白联合检测对糖尿病早期肾损伤的诊断价值及其监控作用。方法用干化学法和速率散射比浊法分别检测105例不同病程糖尿病患者及100名健康对照者的尿蛋白定性和尿微量蛋白微量白蛋白(MA)、α1-微球蛋白(α1-M)、β2-微球蛋白(β2-M)],对检测结果进行统计分析。结果糖尿病组尿蛋白定性及尿微量蛋白测定的结果均明显高于健康对照组,比较有显著性差异;糖尿病组中,尿蛋白定性阳性率只有23.8%,尿MA、α1-M、β2-M阳性率分别为56.2%、64.8%、27.3%;尿MA和α1-M联合检测总阳性率为77.1%,与单测尿MA或α1-M阳性率比较有显著性差异(P<0.05);尿蛋白定性和尿MA,尿α1-M联合检测时,105例糖尿病者尿蛋白定性阴性中有53.3%MA或尿α1-M阳性;糖尿病者随病程增加尿蛋白定性、尿MA和α1-M的阳性检出率不断增高,尿MA和α1-M水平不断升高。结论尿蛋白定性阴性不能排除糖尿病早期肾损伤;尿微量蛋白联合检测对发现糖尿病早期肾损伤有重要意义。

Abstract:Objective To observe the effect of monitoring diabetes nephropathy by the examination of diabetes patient`s urine.Methods Detecting blood glucose at blank state,qualitative urine protein,urine MA andα1-M of 105 cases of diabetes patients in different progress of the disease and compared with 100 health control cases,and doing statistics and analysis on the detecting result.Results Qualitative urine protein,urine MA,urine α1-M and bloood glucose of hungry state were all detected to be higher in diabetes group than that in health control group ,and the difference have statistic significance.In diabetes group,positive rate of urine protein, urine MA and urineα1-M are 23.8%,56.2% and 64.8% respectively.Total positive rate of combining detection of urine MA and urineα1-M is 77.1%.Urine MA or α1-M is positive whose qualitative urine protein is negative in 105 cases of diabetes by combing detection of qualitative urine protein,urine MA and α1-M .The positive rates levels of qualitative urine protein,urine MA andα1-M are becoming higher continuously,as disease progressing of diabetes patients.Conclusion The negative result of qualitative urine protein can not exclude early nephropathy of diabetes.Union detection of urine MA and urine α1-M is of great importance for early discovery disbetes nephropathy. 【
