无创双水平正压通气治疗COPD合并Ⅱ型呼吸衰竭的临床观察及护理

目的：观察BiPAP呼吸机对治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的疗效。方法：将56例慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者进行随机分组，采用无创双水平正压通气（BiPAP呼吸机）辅助通气28例为治疗组；采用鼻导管低流量氧气吸入28例为对照组。两组同时给予抗感染、解痉、平喘、化痰等常规治疗并辅以积极护理措施。观察两组患者的外周血氧饱和度及动脉血气分析结果及临床体征变化。结果：对照组患者在血气分析结果、血氧饱和度提示有轻度改变，而治疗组在血气分析结果、血氧饱和度有显著改善，两者有显著性差异（P〈0．01）。结论：无创双水平正压通气（BiPAP呼吸机）对治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的疗效显著，对低氧血症和C02潴留改善明显，可提高患者的生活质量。

Clinical observation and nursing for non-invasive positive pressure ventilationto treat the COPD concurrent II breath failure clinical.

Objective：To observe the effect of Bi - PAP breath machine to treat the chronic disease （COPD） concurrent Ⅱ breath failure. Methods：56 patients with COPD concurrent Ⅱ breath failure were divided into two groups at random,28 cases in each groups. The treatment group was given non - invasivu positive pressure ventilation ,the control group was given sustained low rate of flow attracts oxygen. Give them convention curing and assisting the active nursing to routine such as resisting,relieve of convulsions et al. Observed blood gas analysis and clinical change about two groups. Results：The control group have a little change, the treatment group were improved greatly, there was significant difference in blood gas analysis and SpO2 （P 〈 0.01 ）. Conclusion：The COPD concurrent Ⅱ breath failure uses the Bi - PAP breath machine to treatment and improvement the low oxygen blood sickness and CO2 keeps to be obvious,and the effect is clear may improve patients＇survival quality.
