漫射光成像技术研究针刺治疗中风疗效比较

目的:比较体针与头体针联合治疗在中风病患者脑内的成像差异,为两者的临床疗效差异提供影像学依据.方法:将20例中风患者随机分为体针组、头体针组,每组10例.体针组采取单纯体针治疗,根据患者症状选取不同穴位,半身不遂取手三里、肩髎、环跳、夹脊等,面瘫取风池、下关、颧髎等,失语取风府、廉泉等；头体针组采用头针和体针联合治疗,体针取穴同上,头针取相应运动区、感觉区、足运感区等.采用近红外漫射光成像(NIRS)系统,检测并评估针刺治疗前后大脑局部脑血流量即时变化,并评价两组疗效.结果:体针组总有效率为90.0％(9/10)、基本痊愈率为30.0％(3/10),头体针组分别为100.0％(10/10)、50.0％(5/10),头体针组临床疗效优于头针组(P＜0.05).两组成像结果显示,体针组与头体针组的前额大脑皮层血流量在针刺后均有不同程度增加(P＜0.05,P＜0.01)；针刺20、30 min后,头体针组较体针组脑部血流量改善更明显(均P＜0.05).结论:针刺均可显著促进脑皮层血流量及血氧饱和度增加,其中头体针联合针刺效果优于单纯体针治疗.
