再生障碍性贫血患者T淋巴细胞亚群与免疫球蛋白的变化及其临床意义

目的　探讨再生障碍性贫血 (再障 )患者T淋巴细胞亚群与免疫球蛋白的变化及其临床意义。方法　对 2 2例再障患者 (再障组 )进行外周血T淋巴细胞亚群及其血清免疫球蛋白检测 ,并与 18例门诊健康体检者 (对照组 )进行比较。结果　再障组CD+ 4 、CD+ 4 /CD+ 8、IgG、IgA、IgM水平比对照组明显降低 (P均 <0 .0 1) ,但CD+ 8明显升高 (P <0 .0 1) ;大部分患者CD+ 3 正常。再障组治疗后 15例有效 ,其IgG、IgA、IgM、CD+ 4 、CD+ 4 /CD+ 8比值升高至正常或接近正常 ,CD+ 8明显降低至正常或接近正常 ,与治疗前相比差异有显著性 (P <0 .0 1或 0 .0 5 )。结论　再障患者发病与免疫功能异常密切相关 ,不仅存在细胞免疫功能的异常 ,也存在着体液免疫功能的异常。用免疫抑制剂治疗有效者 ,T淋巴细胞亚群、免疫球蛋白水平可恢复正常或接近正常

The changes and clinical significance of T-lymphocyte subsets and immunoglobulin in patients with aplastic anemia

Objective To investigate the ch an ges and clinical significance of T-lymphocyte subsets and the immunogloblin in p atients with aplastic anemia. Methods The perip heral T-lymphocyte subpopulation and the serum immunoglobulin in 22 patients wit h aplastic anemia were measured and compared to those in 18 normal individuals (control group). Results The CD + 4, CD+ 4/CD+ 8 , I gG, IgA, IgM levels in patients with aplastic anemia decreased significantly tha n those in the control group (all P<0.01), but CD + 8 elevated significantly ( P<0.01); the CD+ 3 in majority patients were in normal level. After treat ment, 15 of the patients with aplastic anemia recovered, the IgG, IgA, IgM, CD + 4, CD+ 4/CD+ 8 elevated to a normal level or near the normal level, CD + 8 lower to a normal level or near the normal level, in comparison to those before treatment, there were statistically significant difference (P<0.01 or 0.05). Conclusion The attack of aplastic anemi a is closely related to the abnormality of immunologic function, not only the ce lls immunologic function is abnormal, but also is the humoral immunity competenc e. The lymphocyte subpopulation, immunoglobulin levels can recover to a normal l evel or near a normal level in patients who received the treatment of immunosupp ression agent and with good results.
