肝血管瘤的影像学对比研究

摘    要:目的探讨肝血管瘤在超声、CT及MRI的影像特点,比较不同检查方法对病灶的敏感性及特异性.方法经手术病理及血管造影证实的40例肝血管瘤病人,均进行超声,CT及MRI检查.对不同检查方法的影像表现及检出率进行比较,并对敏感性及特异性进行统计学处理.结果超声,CT及MRI对肝血管瘤的敏感性为70%、92.5%、95%,特异性为68.4%、89.5%、94.7%,不同方法之间的敏感性有显著差异(P<0.005),MRI对肝血管瘤的敏感性高于超声及CT.结论超声可做为肝血管瘤的首选方法,采用螺旋CT或/和MRI在定性方面上加以证实,不同方法之间的联合运用,可提高对肝血管瘤的诊断水平.

Abstract:Objective To inquire into the image characteristics of hepatic hemangioma shown by the Ultrasound, CT, and MRI and compare the sensitivity and specificity of the foci in different. Methods 40 patients with hepatic hemangioma proved by pathological specimens remove from operation and angiography received the examinations of ultrasound,CT and MRI.Then the image manifestations and detectable rates found with different method were compared,and the sensitivity and specificity were statistically analyzed. Results The sensitivity of Ultrasound,CT and MRI to hemangioma of liver was 70%, 92.5% and 95%, whereas the specificity was 68.4%,89.5% and 94.7%.The sensitivity detected by diverse means were different (P<0.005). It was also found that the sensitivity of hepatic hemangioma by MRI were higher than those by Ultrasound and CT. Conclusion Ultrasound was the first choice of all the methods when hepatic hemangioma was detected and at the same time, spiral CT scanning and MRI can be used to reconfirm the disease. It was necessary to combine different methods with each other so as to raise the diagnostic level of hemangioma of liver.
