氯氮平联合阿立哌唑治疗男性难治性精神分裂症对照研究

目的 探讨氯氮平联合阿立哌唑治疗男性难治性精神分裂症的临床疗效和安全性.方法 将64例男性难治性精神分裂症患者随机分为两组各32例,研究组口服氯氮平联合阿立哌唑治疗,对照组单用氯氮平治疗,观察8周.于治疗前及治疗2周、4周、8周末采用阳性与阴性症状量表、副反应量表评定临床疗效和不良反应. 结果治疗4周末起,两组阳性与阴性症状量表总分及各因子分均较治疗前有显著下降（P〈0.05或0.01）;治疗8周末,研究组阳性症状分较对照组下降更显著（P〈0.05）;研究组显效率为61.3%、有效率为90.3%,对照组分别为36.7%、83.3%,研究组显效率显著高于对照组（χ2=3.699,P〈0.05）,总有效率差异无显著性（χ2=1.618,P〉0.05）.研究组嗜睡、体质量增加等不良反应发生率显著低于对照组（P〈0.05）,而静坐不能、心动过速发生率显著高于对照组（P〈0.05）. 结论 氯氮平联合阿立哌唑治疗男性难治性精神分裂症疗效显著,安全性高,优于单用氯氮平治疗.

A control study of clozapine plus aripiprazole in the treatment of male treatment-resistant schizophrenia

Objective To explore the clinical efficacy and safety of clozapine plus aripiprazde in the treat- ment of male treatment-resistant schizophrenia （TRS）. Methods Sixty-four TRS patients were randomly assigned to two. groups of 32 ones each, research group took orally clozapine plus aripiprazole and control group did clozapine only for 8 weeks. Clinical efficacies were assessed with the Positive and Negative Syndrome Scale （PANSS） and adverse raections with the Treatment Emergent Symptom Sacle （TESS） before treatment and at the end of the 2nd , 4th and 8th week. Results Since the end of the 4th week, total and each factor scores of the PANSS of both groups lowered more significantly compared with pretreatment （P〈0.05 or 0.01） ; at the end of the 8th week, positive symptoms＇ scores lowered more significantly in the research than in the control group （P〈0. 05）; obvious effective and effective rates were 61.3% and 90. 3% in the research and 36. 7% and 83. 3% in the control group respectively, there was significant differences in obvious effective rates （X2： 3. 699, P〈0.05） and no in total effective rates （χ2= 1. 618, P〈 0.05）. Incidences of lethargy and increased body weight were significantly lower （P〈0.05） and those of akathisia and tachycardia higher （P〈0. 05） in the research than in the control group. Conclusion Clozap- ine plus aripiprazole is superior to clozapine only in the treatment of male treatment-resistant schizophrenia.
