慢性阻塞性肺疾病急性加重期患者间断使用无创通气的效果评价

目的探讨间断无创正压通气治疗慢性阻塞性肺疾病急性加重期（AECOPD）的疗效。方法将我院自2009年2月至2010年2月收治的50例AECOPD患者,随机分为观察组和对照组,每组25例,对照组采用常规治疗,观察组在对照组基础上采用间断无创正压通气,比较两组患者治疗后肺功能、动脉血氧分压及动脉二氧化碳分压及疗效。结果观察组患者中显效20例,有效2例,总有效率为88.0%,显著高于对照组的72.0%,P〈0.05。且观察组的肺功能、动脉血氧分压及动脉二氧化碳分压均较对照组有明显改善,P〈0.05。结论无创通气可迅速改善肺部气体交换,缓解AECOPD患者的缺氧状况,增强肺功能,提高患者的生活质量。

Evaluation of discontinuous noninvasive ventilation for patients with acute exacerbations of chronic obstructive pulmonary disease

【Objective】 To explore the effect of discontinuous noninvasive positive pressure ventilation for patients with acute exacerbations of chronic obstructive pulmonary disease（AECOPD）.【Methods】 From February 2009 to February 2010,50 cases of AECOPD were randomly divided into two groups.25 cases of control group received conventional therapy,and the experience group including 25 patients were treated with discontinuous noninvasive positive pressure ventilation besides from conventional therapy.Pulmonary function,PaO2 and PaCO2 of post-treatment and curative effect were compared between two groups.【Results】 As for the experience group,20 cases were significantly effective and 2 cases were effectual with a total cure rate of 88.0%.The total cure rate of the experience group was much higher than that of control group（72.0%） with significantly difference（P0.05）.Pulmonary function,PaO2 and PaCO2 of post-treatment in experience group were superior to that in control group（P0.05）.【Conclusion】 Discontinuous noninvasive positive pressure ventilation for AECOPD can improve gas exchange,anoxia symptoms,pulmonary function and life quantity of the patients.
