针刺联合蠲痹通瘀汤治疗强直性脊柱炎临床观察

目的观察针刺联合蠲痹通瘀汤治疗强直性脊柱炎(AS)的临床疗效及安全性。方法将64例AS患者随机分为2组,对照组32例应用针刺治疗,治疗组32例应用针刺联合蠲痹通瘀汤治疗。2组均治疗4周。观察2组治疗前后Bath强直性脊柱炎功能指数(BASFI)评分、Bath强直性脊柱炎病情活动指数(BASDAI)评分及主要症状量化分级评分变化,比较2组治疗中安全性。结果 2组治疗后BASFI评分、BASDAI评分及主要症状量化分级评分均较本组治疗前降低(P0.05,P0.01);且治疗组治疗后BASFI评分、BASDAI评分及主要症状量化分级评分均低于对照组治疗后(P0.05)。2组治疗前后丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、血清肌酐(Cr)及尿素氮(BUN)比较差异均无统计学意义(P0.05),2组治疗后ALT、AST、Cr及BUN比较差异均无统计学意义(P0.05)。结论针刺联合蠲痹通瘀汤在改善AS患者主要症状,降低BASFI评分及BASDAI评分方面优于针刺组,且对肝肾功能无明显影响。

Clinical observation of Acupuncture combined with Juanbi-tongyu decoction on ankylosing spondylitis

Objective To observe the effects and safety of acupuncture combined with juanbi-tongyu decoction on the treatment of ankylosing spondylitis (AS).Methods 64 AS patients were randomly divided into two groups.The control group (n=32) was treated by acupuncture, the treatment group (n=32) was treated by acupuncture combined with juanbi-tongyu decoction, continuously treatment for 4 weeks.The scores of Bath ankylosing spondylitis functional index (BASFI), Bath ankylosing spondylitis disease activity index(BASDAI)and the quantification grade of main symptoms before and after treatment were observed in two groups.The safety was compared between two groups.Results The scores of BASFI, BASDAI and the quantification grade of main symptoms after treatment were decreased in two groups (P<0.05, P<0.01), and the decrease in treatment group was lower than that in control group (P<0.05).There was no statistical difference on alanine aminotransferase (ALT), aspartate aminotransferase (AST), serum creatinine (Cr) and urea nitrogen (BUN) between before and after treatment in two groups(P>0.05), and there was no statistical difference on ALT, AST, Cr and BUN after treatment between two groups (P>0.05).Conclusion Acupuncture combined with juanbi-tongyu decoction has better effects on the improvement of main symptoms, can reduce the scores of BASFI and BASDAI than acupuncture group in AS patients,with no significant effect on liver and kidney.functions.
