经皮椎弓根螺钉内固定术治疗侧方应力型胸腰椎爆裂性骨折临床观察

目的观察经皮椎弓根螺钉内固定术治疗侧方应力型胸腰椎爆裂性骨折的临床疗效。方法将50例侧方应力型胸腰椎爆裂性骨折患者随机分为2组,治疗组25例予经皮椎弓根螺钉内固定术治疗,对照组25例予功能锻炼加中药消髓化核汤保守治疗。治疗前及治疗14 d、3个月、6个月后,(1)比较2组患者疼痛视觉模拟评分法(VAS)评分变化;(2)测量2组患者Cobb角变化;(3)比较2组患椎椎体高度变化。结果2组治疗14 d后VAS评分比较差异有统计学意义(P0.05),治疗组低于对照组。2组治疗14 d、3个月、6个月后Cobb角比较差异均有统计学意义(P0.05),治疗组Cobb角小于对照组。2组治疗14 d、3个月、6个月后患椎高度比较差异均有统计学意义(P0.05),治疗组患椎高度高于对照组。结论经皮椎弓根螺钉内固定术治疗侧方应力型胸腰椎爆裂性骨折的临床疗效优于保守治疗。

Clinical observation of operation on the treatment of thoracolumbar burst fracture with lateral stress type

Objective To observe the clinical effects of operation on the treatment of thoracolumbar burst fracture with lateral stress type.Methods 50 subjects with lateral stress type thoracolumbar burst fracture were randomly divided into two groups.25 subjects in operation group were received operative treatment, and 25 subjects in conservative group were received conservative treatment.The changes of visual analogue scale (VAS) score, Cobb's angle and affected vertebral height among before, 14d, 3 and 6 months after treatment were observed.Results The VAS score 14 d after treatment in treatment group was lower than that in control group, with statistical difference (P<0.05).The Cobb's angle 14d, 3 and 6 months after treatment in treatment group were lower than those in control group at the same period, with statistical differences (P<0.05).The affected vertebral height 14d, 3 and 6 months after treatment in treatment group were higher than those in control group at the same period, with statistical differences (P<0.05).Conclusion Operative treatment has better curative effects than conservative treatment on thoracolumbar burst fracture with lateral stress type.
