肠系膜下动脉根部淋巴结清扫在直肠癌治疗中的意义

目的探讨肠系膜下动脉根部淋巴结清扫在直肠癌治疗中的意义。方法 对47例肠系膜下动脉根部淋巴结清扫的直肠癌根治术进行回顾性总结。结果 肿瘤直径大于6 cm者肠系膜下动脉根部淋巴结转移率为28.0％,肿瘤直径小于6 cm者转移率为7.2％,两者有显著差异(P<0.05);肿瘤位于腹膜返折以上者肠系膜下动脉根部淋巴结转移率为27.2％,位于腹膜返折以下者转移率为7.4％,两者有显著差异(P<0.05)。共完成随访36例,复发率为5.56％。结论 肠系膜下动脉根部淋巴结清扫,可以有效降低直肠癌术后复发率。

Significance of Clearing Peripheral Lymph Node of Inferior Mesenteric Arterial Root in Restal Cancer Treatment

Objective To evaluate the clinical significance of clearing peripheral lymph node of inferior mesenteric arterial root in restal cancer treatemt. Methods 47 cases with clearing peripheral lymph node of inferior mesenteric arterial root in restal cancer operation were analyzed retrospertively. Results In 47 cases, the tumor which was more than 6cm was 28.0 % of lymph node metastasis rate of inferior mesenteric arterial root, the tumor which was less than 6cm was 7.2%. There was significantly difference between them ( P < 0.05 ) . Tumors on peritoneal reflection were 27.2 % of lymph node metastasis rate of inferior mesenteric arterial root, tumors under peritoneal reflection were 7. 4%. There was significantly difference between them(P < 0.05 ) .36 cases were followed and the recurrence rate was 5.56% . Conclusions Clearing peripheral lymph node of inferior mesenteric arterial root can reduce the recurernce rate of postoperation.
