中、老年维持性血液透析患者死亡原因对比分析

目的探讨老年维持性血液透析患者的死亡原因,提出防治措施.方法回顾分析2000年1月至2010年4月维持性血液透析48例死亡病例的临床资料,并分为老年、非老年组对比分析.结果老年维持性血液透析患者死亡原因以感染最高,其次为心功能衰竭.年龄、透析不充分和糖尿病、透析间期体重增加量过多、低白蛋白血症等是死亡危险因素,透析间期体重增加量过多以及低白蛋白血症分别是透析患者死亡的独立危险因素.结论防治原发病,改善营养状况,纠正低白蛋白血症,增强机体抵抗力,给予充分的血液透析,控制干体重和透析间期体重增加量是提高老年维持性血液透析患者生存率和生存质量的关键.

Comparative Analysis of Death Cause in Middle-aged and Elderly MHD Patients

Objective To investigate the cause of death in elderly maintenance hemodialysis（MHD） patients,and to find prevention measures.Methods The clinical data of 48 death cases of MHD patients in our hospital from January 2000 to April 2010.The 48 death cases of MHD patients were divided into middle-aged group and elderly group,and the comared the cause of death between the two groups.Results The first cause of death in elderly MHD patients was infection,followed by heart failure.Age,diabetes,and inadequate dialysis,dialysis interphase excessive weight and low serum albumin levels were death risk factors,dialysis interphase excessive weight and low serum albumin levels were independent risk factors.Conclusion Preventing primary disease,improving nutrition,correcting hypoalbuminemia,improving body resistance,giving adequate dialysis,and control of dry weight and weight gain between dialysis are key measures to improve the survival rate and life quality of elderly MHD patients.
