神经阻滞治疗偏头痛75例临床分析

目的观察利多卡因和醋酸泼尼松龙注射液行神经阻滞疗法治疗偏头痛的临床效果.方法 2008年3月至2010年3月昆明医学院第一附属医院微创神经外科对75例偏头痛患者利用利多卡因和醋酸泼尼松龙注射液根据头痛的不同部位行眶上神经、耳颞神经、枕大神经阻滞治疗,随访6个月,以视觉模拟评分法（VAS）评定疗效,并对偏头痛的发病机制、神经阻滞的解剖基础及临床应用复习相关文献.结果治疗后6个月,75例患者中优72例,良3例,其治愈率为96%,有效率为100%.结论利用利多卡因和醋酸泼尼松龙注射液行神经阻滞治疗偏头痛经济、安全可行,具有创伤小、恢复快、疗效可靠及并发症少等优点.

Clinical Analysis of Nerve Block in the Treatment of Migraine

Objective To observe the therapeutic effect of nerve block with lidocaine and prednisolone acetate injection on migraine.Methods A total of 75 patients with migraine underwent supraobital nerve block,auriculotemporal nerve block and great occipital nerve based on different areas of migraine with lidocaine and prednisolone acetate injection from March 2008 to March 2010 in the 1st Affiliated Hospital of Kunming Medical University.After treatment,patients were followed up for 6 months to evaluate the efficacy by visual analog scale（VAS）.We also reviewed the pathogenesis of migraine,anatomical basis for nerve block in treatment of migraine and clinical application.Results After 6 months of treatment,excellent efficacy was noted in 72 patients,good in 3 patients in 75 patients.The cure rate was 96% and the effective rate was 100%.Conclusions It is feasible and safe to use nerve block with lidocaine and prednisolone acetate injection to treat migraine.The treatment has few complications,soon recovery and confirmed efficacy without a dissection balloon.
