刍制硅胶管鼻后麻醉用于纤维支气管镜检查的效果观察

目的 探讨自制硅胶管鼻后麻醉在纤维支气管镜（纤支镜）检查中的效果.方法 纤支镜检查患者98例分为A组50例,B组48例;A组采用1%丁卡因咽喉喷雾麻醉联合环甲膜穿刺2%利多卡因注入麻醉,B组采用1%丁卡因咽喉喷雾麻醉联合自制硅胶管2%利多卡因鼻后滴入麻醉.比较两组麻醉后血压、心率变化及纤支镜过声门后气管见出血发生率、术中体动、愿意接受再次检查率.结果 B组麻醉后血压、心率均低于A组（P 〈 0.05）,A组纤支镜过声门后气管见出血发生率、术中体动均高于B组（P 〈 0.05）,A组接受再次检查率低于B组（P 〈 0.05）.结论 自制硅胶管鼻后麻醉较传统环甲膜穿刺麻醉效果好,不良反应少,简单易行,可广泛推广应用于临床纤维支气管镜检查.

Self-made silicone tube postnasal anesthesia effect observed in fiberoptic bronchoscopy

Objective To explore the effect of self-made silicone tube postnasal anesthesia in the check of fiberoptic bronchoscopy. Methods Fiberoptie bronehoseopy in 98 patients were divided into A group of 50 patients and 48 cases of B. Group A with 1% tetracaine sprayed anesthesia on the throat unite thyrocricocentesis 2% lidocaine injection anesthesia, Group B with 1% tetracaine sprayed anesthesia on the throat unite self-made silicone tube postnasal drip anesthesia with 2% lidocaine. Blood pressure changes, heart rate changes, fiberoptic bronchoscopy through the glottis and the incidence of trachea see bleeding, intraoperative body movement and the rate of willing to accept the check again were compared between two groups. Results After the anesthesia, blood pressure and heart rate of Group B were lower than in group A （P 〈 0.05）,the incidence of trachea see bleeding after fiberoptie bronchoscopy through the glottis and intraoperative body movement of Group A were higher than in Group B （P 〈 0.05）,the rate of accepting recheck Group A were lower than that in Group B （P 〈 0.05）. Conclusion It is demonstrated that the effect of self- made silicone tube postnasal anesthesia better than traditional thyrocricocentesis anesthesia, fewer adverse reactions, simple and feasible, can be widely used in clinical fiberoptie bronchoseopy.
