河北省麻疹疫苗初始强化免疫血清学效果抽样分析

摘    要:目的研究河北省首次实施麻疹疫苗(MV)初始强化免疫(catch-up)血清学效果。方法通过抽样,对同一人群采集强化免疫前后双份血清以及对不同人群分别于强化免疫前后采集血清,采用酶联免疫吸附试验(ELISA)检测麻疹IgG抗体。结果强化免疫接种11920936人,接种率为92.30%;强化免疫后麻疹抗体阳性率从90%上升至99.7%以上,抗体几何平均滴度(GMT)从小于1000上升至2000以上,平均增长3倍以上;人群麻疹抗体滴度组成发生了重大变化,低抗体人群比例下降,高抗体人群比例上升;强化免疫后3年内,抗体水平仍保持在较高的水平。结论MV初始强化免疫血清学效果非常显著,极大提高了人群抗体水平,减少了易感人群的积累,使麻疹发病率保持在较低的水平。

Abstract:Objective To study the serological effect of the measles catch-up mass immunization campaign in Hebei Province. Methods The children from 8 months to 14 years old were the target population of the catch-up immunization. The double serum samples of the same population and the single serum sample before and after the catch-up immunization were collected in order to detect the IgG antibody by ELISA. Results A total of 11 920 936 children were given one dose of measles vaccine, and the average cover rate of measles vaccine was 92.30%. The positive rate of IgG antibody reached over 99.70% from about 90%, and the GMT reached over 2 000 from below 1 000, raising for over 3 folds. Great changes had taken place in the population of the antibody titers. The ratio of the lower antibody titers dropped obviously , and the ratio of the higher antibody titers increased notablely. The antibody level of the population kept high 3 years after the catch-up immunization. Conclusion The serological effect of the catch-up immunization is very good. The antibody level of the population reaches high quickly and efficiently. The catch-up immunization preventes the accumulation of the sensitive population. The incidence of measles keeps very low all the time.
