高血压患者尿白蛋白排泄率与动脉粥样硬化危险因子相关关系的研究

摘    要:目的探讨高血压患者尿白蛋白排泄率与动脉粥样硬化危险因子之间的相互关系。方法84例高血压患者依尿有无微白蛋白分为2组:微白蛋白尿组(MALB)与非微白蛋白尿(NMALB)组,测定其体重指数、空腹血糖、血清胰岛素、血脂、脂蛋白及氧化低密度脂蛋白水平,并与正常对照组相比较。结果高血压MALB组与NMALB组或正常对照组比较,年龄、病程、体重指数、空腹血糖、血清胰岛素、血清TG、OX-LDL水平均显著高于NMALB组,而血清HDL-C均显著低于NMALB组(P均〈0.05);经c对数转换后尿白蛋白排泄率(UAER)与病程、舒张压、平均动脉压、体重指数、血清TG、LDL-C、OX-LDL水平均呈显著正相关,与HDL-C呈显著负相关。结论伴MALB高血压患者动脉粥样硬化性心脑血管疾病发病率增加可能与其所伴发肥胖、脂质代谢异常及胰岛素抵抗等多种动脉粥样硬化危险因素有关。

Abstract:Objective: To study the correlation between urinary albumin excretion and atherogenic risk factors in patients with hypertension..Methods: 84 cases with were divided into two groups: microalbuminuria group and nonmicroalbuminuria group.Parameters of BMI,serum lipid,lipoproteins,glucose,insulin and plasma oxidized low density lipoprotein(OX-LDL)were measured.Results: In microalbuminuric group the levels of sernm,TC,TG,LDL-C,glucose,insulin and plasma OX-LDL were significantly higher than in normalbuminuric subjects.Relationship analysis showed that course,DBP,MBP,BMI,serum HDL-C,LDL-C,TGand plasma OX-LDL were statistically associated with logarithmly transformed UAE.Conclusion: In patients of hypertension with microalbuminuria with elevated UAE,the well-established atherogenic risk factors such as obesity,serum TG,LDL-C and insulin are increased in some degree.This probably could explain why these individuals have higher cardiovascular morbidity and mortality.
