健康自我管理模式信息平台在社区老年性高血压防治中的作用

目的：探讨健康自我管理模式信息平台的推广对社区老年性高血压病干预和治疗的效果。方法：选择广州市荔湾区逢源社区300名老年性高血压病患者并采取分层抽样，随机分成两组。观察组150例，在常规西药降压治疗、常规社区高血压慢病管理及常规健康教育基础上同时利用信息平台在社区推广居民健康自我管理模式进行健康自我管理，推广中医药养生保健和中医适宜技术的应用：对照组150例，采用常规西药降压药治疗，常规社区高血压慢病管理及常规健康教育。两纽均3个月为1疗程，连续随访4个疗程。观察对比老年性高血压患者的健康知识知晓率，健康行为形成率，血压控制率和中医药养生保健、中医适宜技术使用率。结果：疗程结束后，观察组健康知识知晓率，健康行为形成率，血压控制率和中医药养生保健、中医适宜技术使用率均高于对照组（P〈0．05）。结论：健康自我管理模式信息平台应用对于社区老年性高血压病患者可以提高其高血压知识知晓率，健康行为形成率，血压控制率和中医药养生保健、中医适宜技术使用率．．

Effects of information platform for self-management of health on prevention and treatment of hypertension in elderly residents

Objective： To determine the effects of information platform for self-management of health on prevention and treatment of hypertension in elderly residents. Methods： Three hundred elderly patients with hypertension in Fengyuan Community of Liwan District of Guangzhou were subjected to stratified sampling and randomly allocated to be treated with conventional anti-hypertensive therapy and health education alone （control group, n = 150）or in combination with the patient-oriented self-management （observation group, n = 150） implemented by an information platform that aimed to promote the use of self-management program and traditional Chinese medicine-based healtbcare and feasible techniques on a community basis. A total of four treatment courses that individually comprised of a 3-month therapy were scheduled. This entailed comparison on the awareness of health-related knowledge of hypertension in the elderly, formation of healthy behaviors, blood control and utilization of traditional Chinese medicine-based healthcare and feasible techniques. Results： Following the treatment courses, the observation group yielded significantly improved awareness of health-related knowledge of hypertension in the elderly, formation of healthy behaviors, reduced blood control and increased utilization of traditional Chinese medicine-based heahhcare and feasible techniques （all P 〈 0. 05 ）. Conclusion： The information platform for self-management of health may effectively improve the awareness of health-related knowledge of hypertension in the elderly, formation of healthy behaviors, reduce the blood control and increase the utilization of traditional Chinese medicine-based healthcare and feasible techniques.
