微创体外循环在成人气管肿瘤外科手术中的应用

摘    要:目的总结气管外科手术中应用微创体外循环的经验。方法 2008年1月至2009年12月,使用常规体外循环器材自行组装微创体外循环管路,按照闭式常温中流量股动静脉体外循环对5例气管肿瘤导致严重气管狭窄患者进行呼吸支持。结果 5例患者提前建立体外循环后,安全度过了由于严重气管狭窄所致麻醉诱导和手术难关,未有明显血流动力学波动和凝血功能障碍。结论充分理解微创体外循环理念,使用现有常规体外循环器材建立微创体外循环,适应气管外科麻醉和手术需要,具有创伤小、支持充分、性能价格比较高的特点。

Abstract:OBJECTIVE To summarize the experiences of using minimal cardiopulmonary bypass for tracheal tumor resection in adult patients.METHODS From January 2008 to December 2009,5 cases with tracheal tumor with severe tracheal stenosis underwent tumor resection with the assistance of minimal cardiopulmonary bypass.The bypass circuits were connected from those disposable devices as routinely clinical used.The bypass management mode was normal temperature,medium flow rate,femoral to femoral bypass in close circuit.RESULTS All of 5 cases were successfully managed by surgical resection of tumor under minimal cardiopulmonary bypass without signs of hemodynamic disturbance and coagulative dysfunction.CONCLUSION Compare with conventional cardiopulmonary bypass,Minimal cardiopulmonary bypass is better in less invasive and less coagulopathy for tracheal surgery.
