老年患者行腹腔镜胆囊切除术的临床分析

目的探讨老年患者行腹腔镜胆囊切除术（LaparoscopicCholecystectomy，LC）的可行性、安全性及手术技术特点。方法回顾分析90例老年患者行LC的临床资料。结果65例（72％）合并不同程度的心血管疾病、慢性呼吸系统疾病或（和）糖尿病；5例（5．5％）有腹部手术史。Lc中转开腹2例（2．2％），LC平均手术时间25—110min，术中平均出血20—100ml，术中无损伤，术后发生并发症3例（3．3％），其中肺部感染l例，局限性腹腔积液1例，泌尿系感染l例。90例均治愈出院，无死亡病例，术后平均住院4～13d。结论LC是一种安全可行、创伤小、并发症少、术后恢复快的手术；选择腹腔镜胆囊切除术治疗老年人胆囊良性疾病是安全可行的。

Laparoscopic Cholecystectomy in Aged Patients

Objective To evaluate the feasibility, safety and technical characteristics of Laparoseopic chalecystectomy（LC） in aged patients. Methods LC was performed in 90 aged patients, and clinical data were analyzed retrospectively. Results Sixtyfive cases（72% ） were with variant degree of combined disease such as cardiovascular diseases,chronic respiratory diseases or/ and diabetes mellitus ; five cases （5.5 % ） with history of previous intra-abdominal operations ; two cases （ 2.2 % ） converted to an open procedure. The average operation time of LC was 25 to 110 min,the average blood was 20 to 100ml. There was no injury dur- ing the operation. Three cases （3.3 % ） developed postoperative complications：respiratory tact infection in 1 cases,local intraperitoneal fluid collection in 1 case, and urinary tract infection in 1 case. All patients recovered favorably and discharged from hospi- tal 4 to 13 days after LC. Conclusion Laparoscopic cholecystectomy is a feasible and safe minimal invasive operation with the advantages of safety, less complications and faster recovery for aged patients with benign gallbladder diseases.
