低血清总胆红素水平预测冠心病发病危险的探讨

目的确定低血清总胆红素（TBIL）水平与冠心病（CHD）之间关系，对低血清总胆红素水平预测冠心病发病危险参考值范围的界定进行探讨。方法使用RXL和BAYER1650生化分析仪测定冠心病患者、正常对照组及正常体检人群的血清总胆红素；使用病例一对照研究设计的2×2表资料及对应的统计学分析方法对数据和资料进行分析，使用百分位数法并结合不同血清总胆红素暴露水平与冠心病的关系来界定低血清总胆红素水平预测冠心病发病危险的参考值范围和危险水平临界值。结果冠心病患者血清总胆红素水平为（8．98±4．26）umol/L，正常对照组血清总胆红素水平为（13．61±5．78）umol／L，两组血清TBIL水平有显著性差异（u=6．12，P〈0．01）；正常体检人群血清总胆红素水平为（13．89±5．82），低血清总胆红素水平预测冠心病发病危险的参考值范围为TBIL≤10．80umol／L。结论低血清TBIL水平与冠心病的发病之间存在相关性．初步得到本实验室低血清总胆红素水预测冠心病发病危险的参考值范围。

The study of predicting cornary heart disease with low level of total bilirubin

Objective To study the relationship between low level of serum total bilirubin（TBIL） and coruary heart disease（CHD）, and establish the reference value for predicting CHD with low level of TBIL. Methods RXL and BAYER1650 biochemical analyzer were used to detect serum total bilirubin of CHD, healthy people and the persons for physical exanimation. All of data was analyzed by some related statistic methods such as 2×2 table＇s and odds ration to determine the reference value for predicting CHD. Results The TBIL level of CHD was 8.98±4.26umol/L and the healthy people was 13.61±5.78umol/L. Both was significantly difference （u=6.12, P〈0.01）. The TBIL level of the persons for physical exanimation ,was 13.89±5.82umol/L. Lower than 10.80umol/L of TBIL was possible to prompt CHD. Conclusion Low level of TBIL is related to CHD, and lower than 10.80umol/L can be used to predict CHD.
