应用亚砷酸艾迪治疗晚期肝癌临床分析

目的总结应用亚砷酸、艾迪（Ai—di）注射液治疗晚期原发性肝癌的临床经验，探讨两者协同作用治疗晚期肝癌的疗效。方法将本组37例患者随机分为治疗组和对照组。治疗组19例晚期原发性肝癌患者应用亚砷酸联合艾迪注射液进行治疗，艾迪注射液50ml＋5％葡萄糖500ml治疗14d后加用0．1％亚砷酸注射液10ml＋5％葡萄糖盐水500ml，1次／d，共用28d。对照组18例，予0．1％亚砷酸注射液10ml＋5％葡萄糖盐水500ml，1次／d，共用14d。观察治疗前后患者临床症状，Kamofsky评分，B超、CT结果，实验室指标等变化。结果本治疗组19例患者全部完成治疗计划，治疗期间无死亡病例。其中16例临床症状减轻，Karnofsky评分升高，B超、CT结果提示肿块大小无明显变化，血常规提示白细胞、血小板降低，肝功能改善，AFP略有降低，肾功能元明显变化，T细胞亚群CD3、CD4、CD8、CD4／CD8升高，不良反应较对照组轻。结论亚砷酸是失去手术机会的晚期原发性肝癌化疗的一种治疗方法，加用艾迪注射液可能有协同作用，能降低不良反应，改善症状，提高生活质量。

Effect of arsenic trioxide and AiDi in the treatment of advanced stage primary liver cancer

Objective To study the clinical experience of arsenic trioxide and AiDi in the treatment of advanced stage primary liver cancer, and explore their cooperative effects on liver cancer. Methods 37 cases with ad vanced primary liver cancer were randomly divided into treatment group and control group. Treatment group （ 19 cases） with advanced primary liver cancer were treated with arsenic trioxide and AiDi injection. Treatment with AiDi at single dose of 50ml/d and 5% glucose injection 500ml/d were used for 14 days, then 0.1% arsenic trioxide injection at single dose of 10ml/d and 5% glucose injection 500ml/d were added for 28 days. Control group （18cases） were treated with 0. 1% arsenic trioxide injection at single close of 10ml/d and 5% glucose injection 500ml/d for 14 days continuously. Clinical symptoms, Karnofsky scores, B -type ultrasonic inspection, CT and laboratory index were compared between prior and pest treatment. Results 19 cases in treatment group achieved progress without death cases in treatment period. 16 cases obtained remission, and Karnofsky scores increased. The tumor size had no noticeable change, and leucocyte and plastocyte decreased. Liver function was improved. AFP slightly decreased. Renal function had no noticeable change. CD3、CD4 、CD8、CD4/CD8 of T lymphocyte subsets increased. The adverse effect was fewer in the treatment group than that in the control group. Conclusion Ai - di injection in combination with arsenic trioxide has marked cure effect and can improve life quality.
