经腹后纵隔贵门癌根治术

目的探讨经腹后纵隔施行贲门癌根治术.方法对1993～2003年经腹采用管状吻合器进行纵隔内食管胃(空肠)吻合的134例进行回顾性分析.结果根治性切除贲门癌时,在癌灶以上5cm切断食管,无切缘癌残留;无手术死亡,2例吻合口瘘,3例吻合口狭窄;并发症低于开胸手术.结论经腹扩大后下纵隔视野,有利于施行贲门癌根治术、纵隔内食管胃(空肠)机械吻合术,能预防并发症的发生.

Transabdominal Posterior Mediastinum Radical Operation for Cadial Carcinoma

Objective: To evaluate the efficacy of transabdominal posterior mediastinum radical operation in the treatment of cardial carcinoma. Method: From 1993 to 2003,134 patients were treated with transabdominal intrame-diastinal esophagogastrostomy or esophagojejunostomy Jia the end-to-end anastomosis (EEA)stapler. Result: In radical resection of cadial carcinoma,the average length of resected esophagus above the carcinoma was about 5 cm,there was no remnant at the margin.There was no operative complication of death,2 anas tomotic leakage,3 anastomotic stenosis,which was lower than those in the thoraco tomy. Conclusion: The transabdominal approach makes a clear operative field in postero-inferior mediastinum,it is helpful to performi radical resection of cardial carcinoma and intramediastinal esophagogastric or esophagojejunal mechanical anastomosis,and prevent complica tions.
