益气活血通络方对老年高血压病气虚血瘀证病人中医证候及甲襞微循环的影响

目的观察益气活血通络方对老年高血压病气虚血瘀证病人中医证候及甲襞微循环的影响。方法将80例老年高血压病气虚血瘀证病人随机分为治疗组和对照组，各40例。治疗组口服中药益气活血通络方和苯磺酸氨氯地平片，对照组口服苯磺酸氨氯地平片，疗程均为8周。观察两组降压疗效、安全性、中医证候疗效及治疗前后甲襞微循环积分值的变化。结果治疗组降压总有效率为95．0%，对照组为87．5％，两组比较差异无统计学意义（P〉O．05）。治疗组和对照组的不良反应发生率分别为2．5％、7．5％，两组比较差异无统计学意义（P〉O．05）。治疗组中医证候疗效总有效率为95．0％，对照组为72．5％，两组比较差异有统计学意义（P〈O．01）。治疗组甲襞微循环形态积分、流态积分、管周状态积分和总积分均明显减少（P〈O．05或P〈O．01），对照组仅形态积分值与治疗前比较有统计学意义（P〈O．05）。结论益气活血通络方对老年高血压病气虚血瘀证病人有较好的疗效，且对病人的微循环障碍有明显的改善作用。

Effect of Yiqi Huoxue Tongluo Formula on TCM Syndrome and Nailfold Microcirculation in Elderly Hypertensive Patients

Objective To explore the effect of Yiqi Huoxue Tongluo formula（YHTF） on traditional Chinese medicine （TCM） syndrome and nailfold microcirculation in elderly hypertensive patients with qi - deficiency and blood - stasis syndrome. Methods Eighty patients with elderly hypertension and qi - deficiency and blood - stasis syndrome were randomly divided into treatment group treated with YHTF and amlodipine besylate tablet,and control group treated with amlodipine besylate tablet. Antihypertensive effica cy and safety were evaluated, and integral value of nailfold microcireulation and TCM syndrome were compared before and after treat ment. Results In antihypertensive efficacy,the total effective rate were 95.0% in treatment group and 87.5 ~/00 in Control group, there was no significant difference between the two groups. The incidence of side effect was lower in the two groups. The total effective rate of TCM syndrome in treatment group was 95.0 %, compared with that in the control group （72.5 %）, there was significant differ ence. The form of the capillary, the blood flow, the perivascular condition and the total score reduced in the treatment group after treatment, the difference was statistically significant compared with that before treatment. Integral value of the form of the capillary was reduced in the control group,there was significant difference compared with that before treatment. Conclusion YHTF had better therapeutic effect in elderly hypertensive patients with qi- deficiency and blood- stasis syndrome, and could effectively improve hy pertensive microcirculation obstacle.
