2011年成都某综合医院呼吸内科门诊疾病谱调查

目的通过调查我院呼吸内科门诊疾病构成,初步预测成都市综合医院呼吸内科门诊疾病谱。方法随机收集四川省人民医院2011年1月1日至2011年12月31日呼吸内科门诊电子病历,根据人民卫生出版社《内科学》第七版并结合初诊医生的初步诊断,获得各种疾病的构成。结果共收集呼吸内科门诊患者14900例,平均年龄（46.3±17.2）岁,男女性患者比例基本相当,23～48岁及53～68岁为呼吸内科门诊就诊的高峰年龄段。呼吸内科门诊最常见的十大病因依次为：急性气管-支气管炎、胸痛、慢支炎、COPD、亚急性咳嗽、慢性咳嗽、上呼吸道感染、肺炎、各种胸闷气紧、支气管哮喘及咯血。部分疾病存在季节性波动。结论本研究首次预测成都市综合医院呼吸内科门诊疾病谱,具有一定代表性,亦提示我们应当警惕和重视的疾病和人群,这也为呼吸系统疾病的防治提供了理论依据。

A Study of Disease Spectrum of Respiratory Outpatients in A General Hospital in Chengdu City in 2011.

Objective To carry out preliminary prediction about disease spectrum of respiratory outpatients in general hospitals in Chengdu city by exploring the constitution of disease among respiratory outpatients in our hospital in 2011. Methods Electronic records of respiratory outpatients from January first, 2011 to December 31, 2011 of Sichuan Provincial People＇s Hospital were randomly recruited and analyzed. The constitution of various diseases was gained based on the Internal Medicine 7th edition published by People＇s Medical Publishing House and doctors＇ primary diagnosis. Results A total of 14900 respiratory outpatients were collected. The mean age was（46.3 ± 17.2）years old. Numbers of males and females were almost equal. Age groups from 23 to 48 years old and from 53 to 68 years old had peak attendance rates. The ten most common diseases of respiratory outpatients were as following： acute tracheal-bronchitis, chest pain, chronic bronchitis/COPD, sub-acute cough, chronic cough, upper respiratory tract infection, pneumonia, chest tightness/breathlessness, bronchial asthma and hemoptysis. The attendances of some diseases fluctuated seasonally. Conclusions Our study was the first trial to predict disease spectrum of respiratory outpatients in general hospitals in Chengdu. To some degree, it has popularity and gives us suggestion that diseases and patient age groups should be paid more attention to. It also provides theoretical bases for prevention and treatment of respiratory diseases.
