女性乳腺超声分型与乳腺肿块良恶性间的相关性

摘    要:目的:利用超声检查对女性乳腺进行分型,并探讨女性乳腺的超声分型与病灶良恶性间的相关性。方法:观察2012年9月至2013年3月,因乳腺肿块于上海交通大学医学院附属瑞金医院行手术治疗的432例患者(498个病灶),所有病例均经病理检查证实,其中恶性病灶162个,良性病灶336个。根据超声图像上病灶侧乳腺腺体组织厚度和结构将乳腺分为腺体型、腺纤维型和脂肪型,并采用χ2检验分析乳腺类型与病灶良恶性间的相关性。结果:腺体型与腺纤维型患者间的病灶良恶性分布差异无统计学意义(P=0.203),而腺体型与脂肪型间、腺纤维型与脂肪型间的良恶性病灶分布差异有统计学意义(P<0.001)。结论:女性乳腺的超声分型与乳腺癌间存在相关性,其中脂肪型乳腺患者的恶性肿瘤发生率最高。

Abstract:Objective: To analyze the correlation of ultrasound classification of female mammary gland with the diagnosis of benign and malignant breast tumors. Methods: Four hundred and thirty-two patientswith 498 breast lesions undergone breast operation in Ruijin Hospital during Sep.2012 to Mar.2013 were enrolled. All lesions were pathologically confirmed, including 162 malignant lesions and 336 benign ones. According to the thickness and structure of the mammary gland tissue, the ultrasound imagings were categorized into 3 types, including glandular type, glandular-fibrous type and fatty type. The correlation of ultrasound classification of female mammary gland with the diagnosis of benign and malignant breast tumors was tested by X2 test. Results: No significant difference was found in malignant/benign distribution between glandular and glandular-fibrous types (P=0.203), but there was a significant difference between glandular type and fatty type, glandular-fibrous type and fatty type (P〈0.001). The fatty type mammary gland had the highest incidence of malignant tumor, and the glandular type had the lowest (X2=46.20, P〈0.05). Conclusions: The ultrasound type of female mammary gland has certain correlation with breast cancer. The highest incidence of the malignant lesion is seen in the fatty type mammary gland.
