富血小板血浆对家兔腰椎后外侧植骨融合作用的研究

目的 研究富血小板血浆(PRP)在家兔腰椎后外侧植骨融合中的作用,为脊柱的融合找到一个新的途径.方法 制备PRP,取家兔24只,取双侧肋骨,行双侧第5、6腰椎横突间的后外侧植骨融合.采用自身对照方法,实验侧取PRP复合肋骨骨粒植入融合区,对照侧取肋骨骨粒植入融合区.术后4、8、12周处死各处死8只动物取脊柱标本.利用影像学、组织学的手段观察植骨区的融合情况,从大体、组织、细胞等方面分析植骨融合情况.结果 PRP复合自体骨组术后4、8、12周各项评分明显优于单纯自体骨组(P＜0.05).实验侧植骨区阻射密度值4、8和12周分别为51±7、69±7、81 ±6；对照侧为33 ±9、40 ±6、50 ±6,2组同时间点比较差异有统计学意义(均P＜0.05).实验侧植骨区放射学评分4、8和12周分别为(3.1±0.8)、(6.9±1.1)、(8.1±1.1)分,2组同时间点比较差异有统计学意义(P均＜0.05).结论 PRP复合自体骨移植对家兔腰椎后外侧植骨融合具有促进作用.

Study on the effect of platelet-rich plasma in the posterolateral fusion of rabbit lumbar

Objective To study the effect of platelet-rich plasma （PRP） in the posterolateral fusion of rabbit lumbar and to provide the new approach for the fusion of spine. Methods PRP was prepared, bilateral ribs were taken from 24 rabbits and lumbar 5 and 6 underwent bilateral posterolateral intertransverse fusion. Using selfcontrol method, the bone particles with PRP was implanted to fusion zone in the experimental side, and the bone particles in the control side. Rabbits were sacrificed after 4,8 and 12 weeks to take the spine specimens. The spine fu- sion were observed by means of histology and imaging to evaluate the situation of fusion from the general, tissues, ceils and other aspects. Results 4,8 and 12 weeks after surgery, the score was significantly better in experimental group than that in control group. The radiopacity density of fusion bone were 51± 7,69 ± 7,81± 6 in experimental group and 33± 9,40 ± 6,50± 6 in control group at 4,8 and 12 weeks after surgery respectively. There was significant difference on the density at any different time between these two groups（P 〈 0.05 ）. Radiology score of fusion bone were （3. 1±0. 8）, （6. 9 ± 1.1）, （8. 1 ± 1.1） in experimental group at 4,8 and 12 weeks after surgery respectively, and there was also significant difference on the score at any different time between these two groups （ P 〈 0.05 ）. Conclusion The autogenous bone with PRP can promote the posterolateral fusion of rabbit lumbar.
