成人小脑髓母细胞瘤的诊断与治疗

目的 探讨成人小脑髓母细胞瘤的临床特点及治疗方法.方法 回顾性分析16例经手术和病理证实的成人小脑髓母细胞瘤病例的临床表现、诊断及综合治疗方法.16例患者均采用显微手术,肿瘤全切14例,次全切2例；16例行手术后放疗,2例加行化疗.结果 随访14例,术后原位复发2例,脊髓转移1例.术后平均生存时间为(70±5)个月,术后5年生存率为68.1％.结论 成人小脑髓母细胞瘤较易误诊,手术全切肿瘤后辅以足够剂量的放疗为主的综合治疗可有效降低复发率和转移率,预后较好.

Diagnosis and treatment of eerebellar medulloblastoma in adults

Objective To discuss the clinical features and treatment of cerebellar medulloblastoma （MB） in adults. Methods A total of 16 patients with cerebellar MB which were confirmed surgically and pathologically were retrospectively studied for the clinical manifestations, diagnosis and comprehensive treatment. Results All patients were treated with microsurgical procedures. Fourteen patients underwent total resection and 2 subtotal resection. The 16 patients were given postoperative radiotherapy and 2 cases received chemotherapy. Fourteen patients were followed up, among them 2 encountered local relapse and 1 occurred spinal cord metastasis. The 5-year surviv- al rate was 68. 1%. Conclusions Sometimes the cerebellar MB in adults can be misdiagnosed. Total resection and adjuvant radiotherapy is effective to reduce the chance of recurrence and metastasis and produces a good prognosis.
