超声造影和超声弹性成像诊断肝脏实性占位的临床价值

摘    要:目的评价超声造影和超声弹性成像在肝脏实性占位中的鉴别诊断价值。方法对60名肝脏实性占位患者的82个实性占位进行研究,应用常规超声、超声造影和超声弹性成像进行检查,并结合手术、穿刺活检病理进行对比分析,评估各检查方法的灵敏度、特异度、阳性预测值、阴性预测值及诊断符合率。结果 82个肝脏实性占位中良性病灶48个,恶性病灶34个。超声造影的灵敏度、特异度、阳性预测值及阴性预测值分别为91.18%、93.75%、91.18%及93.75%,弹性成像的灵敏度、特异度、阳性预测值及阴性预测值分别为88.24%、91.67%、88.24%及91.67%,超声造影联合超声弹性成像的灵敏度、特异度、阳性预测值及阴性预测值分别为97.06%、85.42%、82.50%和97.62%。结论超声造影和弹性成像对肝脏实性占位有较高的鉴别诊断价值,两者联合可提高对该病的诊断率。

Abstract:Objective To evaluate the value between contrast-enhanced ultrasound and elasto-sonography in the diagnosis of solid liver lesions. Methods Sixty patients (82 solid liver lesions) were detected with routine ultrasound, contrast-enhanced ultrasound and elasto-sonography and combined with surgery, pathologic biopsy. The sensitivity, specificity, the positive predictive value, negative predictive value and diagnostic accuracy of each method were evaluated and compared. Results Of the 82 liver lesions, 48 were benign lesions, and 34 were malignant lesions. The sensitivity, specificity, the positive predictive value and negative predictive value were 91.18%, 93.75%, 91.18% and 93.75% for contrast-enhanced ultrasound, 88.24%, 91.67%, 88.24% and 91.67% for elasto-sonography, 97.06%, 85.42%, 82.50% and 97.62% for contrast-enhanced ultrasound combined with elasto-sonography. Conclusion Contrast-enhanced ultrasound combined with elasto-sonography has higher value in the differential diagnosis of solid liver lesions.
