关节镜辅助下微创治疗胫骨平台骨折

目的:探讨关节镜辅助下治疗胫骨平台骨折应用微创手术治疗的方法和临床疗效。方法:自2007年1月～2009年1月在关节镜监视下应用微创技术治疗SchatzkerwⅠ～Ⅳ胫骨平台骨折27例,常规进行膝关节镜检查,对合并伤进行相应的处理后,在关节镜与C型臂机辅助下微创进行骨折复位、植骨及固定,术后CPM功能锻炼。结果:本组平均随诊12个月,骨折均完全愈合,用Lysholm评分标准评定膝关节功能,优19例,良6例,中1例,差1例,优良率为92.3%。结论:关节镜辅助下微创治疗胫骨平台骨折手术创伤少,骨折复位直观,固定可靠,并可实现早期功能锻炼,并发症少,是胫骨平台骨折手术的较理想方法。

Minimally invasive treatment of tibial plateau using arthroscopy

Objective To investigate clinical results of the arthrocopic technique in the treatment of tibial plateau fractures.Method A total of 27 cases of tibial plateau fractures,consisting of 8 cases of type I,10 of II,7 of III,2 of I Vaccording to Schatzker classification,were treated with reduction and screw fixation assisted arthroscopically over a span from 2007 to 2009.The associated intraarticular pathology were treated simultaneously.All of the cases were followed up 12 months in average and post operational complications and function recovery were investigated.Results All of the fractures healed.The rate of excellent and good results was 92.3 % according to Lysholm assessment criterion.Conclusion The reduction and fixation with assistance of arthroscope for the tibial plateau fracture has the advantages of minimal injury,safe operation,accurate fixation,early recovery and convenient treatment of the associated intraarticular pathology at the same time.
