低浓度罗哌卡因鞍麻用于小儿包皮环切术的麻醉效果观察

目的评价低浓度罗哌卡因鞍麻用于小儿包皮环切术的麻醉效果及安全性。方法 120例择期行包皮环切术患儿,随机分为鞍麻组和静脉麻醉组,每组60例。记录术前（T1）,游离冠状沟（T2）,切皮（T3）,术毕（T4）的Bp、HR、SpO2等血流动力学改变;以镇痛镇静评分评价麻醉效果;记录术后清醒时间及并发症。结果与静脉麻醉组相比较,鞍麻组麻醉效果满意,血流动力学改变更为平稳（P〈0.05）;术后清醒时间短（P〈0.05）;术后疼痛、躁动和SpO2下降等并发症明显少于静脉麻醉组（P〈0.05）。结论低浓度罗哌卡因鞍麻用于小儿包皮手术,操作简单、安全,麻醉效果好,不良反应小,是一种安全可行的麻醉方法,与静脉全麻比较具有更多的优势。

Effect observation of saddle anesthesia by low concentration ropivacaine in posthetomy for children

Objective To evaluate the effect and safety of saddle anesthesia by low concentration ropivacaine using in posthetomy for children.Methods 120 children who underwent elective circumcision were randomly divided into two groups： saddle anesthesia group with 60 cases and total intravenous anesthesia group with 60 cases respectively.Recorded how the hemodynamic changed which included Bp,HR,SpO2 et al in pre-surgery（T1）,dissociating coronary sulcus（T2）,skin incision（T3）,end of surgery（T4）,evaluated the anesthetic effect by analgesic sedative score,and recorded postoperative awake time and complications.Results Compared with the total intravenous anesthesia group,the anesthetic effect was satisfactory in saddle anesthesia group,Hemodynamic change in the saddle anesthesia group was more steady（P0.05）,postoperative awake time was shorter（P0.05）,and postoperative pain,dysphoria or decreased SpO2 were obviously fewer（P0.05）.Conclusion Saddle anesthesia by low concentration ropivacaine is easier,more effective,safety and less adverse reaction than total intravenous anesthesia in posthetomy for children.
