重度烧伤患者凝血指标及血小板计数变化的临床意义

目的探讨重度烧伤患者凝血指标及血小板的变化的临床意义。方法用美国贝克曼库尔特公司的全自动凝血分析仪,检测132例重度烧伤患者凝血酶原时间（PT）、凝血酶时间（TT）、活化部分凝血酶时间（APTT）、纤维蛋自原（Fbg）水平;用迈瑞BC-5800无分类血液分析仪检测血小板（PLT）,并选择40例健康体检者作为对照。结果重度烧伤患者PLT、PT、TT、APTT、Fbg水平与对照组比较差异均有统计学意义（P〈0.05）,烧伤程度越重,差异越明显。结论重度烧伤患者存在的凝血系统障碍、血小板和纤维蛋白原升高、纤溶系统功能异常,是严重烧伤后诱发弥漫性血管内凝血（DIC）的重要因素,临床医生应当高度重视。

Clinical significance of coagulant index and changes of platelet count in patients with severe burns

Objective To explore the clinical significance of coagulant index and changes of platelet count in patients with severe burns. Methods Prothrombin time（PT）,thrombin time（TT）,activated partial thromboplastin time（APTT）,fiber protein from the original（Fbg） levels in 132 cases of severe burn were measured by Beckman Coulter＇s automated coagulation analyzer;platelet count（PLT） were measured by Mindray BC-5800 ataxonomic hematology analyzer,at the same time,we selected 40 healthy volunteers as controls to compare. Results The difference of PLT,PT,TT,APTT,Fbg levels between severe burn patients and control group had statistical significance（P0.05）,the more severe burns,the more obvious differences. Conclusion Coagulation system disorders,increasing platelet and fibrinogen and fibrinolytic system dysfunction in patients with severe burn are the important factors of disseminated intravascular coagulation（DIC） after severe burns,clinicians should be highly valued.
