N-乙酰-β-D氨基葡糖苷酶在肾病早查中的临床意义

目的探讨N-乙酰-β-D氨基葡糖苷酶（NAG）检测在肾病早查中的临床意义。方法分析28例IgA肾病患者的临床及病理资料,探讨肾小球及间质病理损害程度与NAG的关系。结果肾小管间质损害情况：肾小管间质损害轻度者占46.43%,中重度者占32.14%,无肾小管间质损害者21.43%。肾小管间质损害与NAG的关系：肾小管间质损害中、重度者尿NAG酶（5.141±1.821）U/（mmol.cr）;小管间质损害轻度者尿NAG酶（1.659±1.485）U/（mmol.cr）;无肾小管间质损害者尿NAG酶（1.447±1.816）U/（mmol.cr）。小管间质中重度损害者同其他两组比较,其尿NAG酶有显著统计学差异,P〈0.05。结论 IgA肾病常合并小管间质损害,肾小管功能中尿NAG酶改变与肾脏小管间质损害程度成对应关系。

Clinical Significance in detecting N-acetyl-beta-D-glucosaminidase activity on early IgA Nephropathy

Objective To investigate the clinical significance in detecting N-acetyl-beta-D-glucosaminidase（NAG） activity in early IgA nephropathy.Methods We collected the clinical and pathological data of 28 cases with early IgA nephropathy and analyzed their relation between the urinary NAG and tubule function,and the degree of pathological lesion of glomerulus or renal interstitial.Results The conditions of renal interstitial lesion in 28 cases： the mild was 46.43%,the middle and severe were 32.14 % and no lesion was 21.43%.The relation between NAG and pathological lesion of renal interstitial： the levels of the urinary NAG in patients with middle and severe lesion was（5.141 ±1.821） U/（mmol·cr）,the level of the urinary NAG in patient with mild lesion was（1.659±1.485） U/（mmol·cr）,the level of the urinary NAG in patient with middle and severe lesion had significant statistical difference compared with other groups（P0.05）.Conclusion The IgA nephropathy usually complicated with renal tubule interstitial lesion.The change of levels of the urinary NAG could be correlated to the degree of pathological lesion of renal interstitial.
