多层螺旋CT重建技术在颌面部外伤中的应用

摘    要:目的探讨多层螺旋CT(MSCT)容积再现三维成像(VRT)、多平面重建(MPR)及最大密度投影(MIP)在颌面部骨折中的应用价值。方法回顾分析65例因颌面部外伤进行MSCT检查并均采用VRT、MPR及MIP对轴扫图像进行三维重建。结果VRT在显示颌面部骨折的类型、位置、范围、碎骨块移位、塌陷,尤其是不规则骨折线的走行等空间信息方面优于多平面重建。MPR在显示深部结构骨折、细小骨折等方面优于容积再现三维成像。MIP对于线性骨折显示清晰。结论 MSCT具有快速扫描、图像分辨率优良和强大的图像后处理功能,MSCT容积再现三维成像及多平面重建对颌面部外伤骨折的定位、定性诊断具有很高价值。MPR、MIP和VRT3种方法相互补充能准确全面地显示颌面部骨折情况,为外科整复手术提供了直观的参考。

Abstract:Objective To study the value of 3 dimensional reconstruction of spiral computed tomography(CT),volume rendering(VR),multiplanar reconstruction(MPR) and maximum intensity projection(MIP) in diagnosing maxillofacial bone fractures.Methods MPR,MIP and VR techniques with spiral CT were performed in 65 cases of different maxillofacial bone fractures for 3 dimensional reconstruction.Results Compared with MPR,VRT was superior in displaying all kinds of maxillofacial bone fractures,location,bone chip displacement,collapse degree and walking direction of irregular fracture line.MPR was better in showing deep structure fracture and tiny fracture.Accurately line type fracture was able to be show in MIP images.Conclusion MSCT has the merits of high speed scan,excellent discrimination and afterward process,and MSCT is of great value in localization and qualitative diagnosis of maxillofacial bone fractured.Combination of MPR,MIP and VR can show the fractures perfectly and provide intuitionistic help for reconstructive surgery.
