无创通气治疗慢性阻塞性肺疾病急性加重期合并呼吸衰竭的临床护理

目的：观察使用无创通气治疗慢性阻塞性肺疾病急性加重期（AECOPD）患者的并发症并总结治疗相关并发症的临床护理措施。方法：收集义乌市中心医院呼吸内科2011年5～10月收治的102例COPD患者,分析使用无创通气治疗后的并发症。结果：大部分患者均出现相应的并发症,消化道症状68例（66.7%）,恐惧心理42例（41.2%）,压迫性损害80例（78.4%）,眼口鼻干燥56例（54.9%）,排痰困难88例（86.2%）,吸入性肺炎30例（29.4%）。加强护理后患者的症状明显好转。结论：无创通气治疗AECOPD患者过程中存在诸多不良反应及并发症,通过积极地护理可以减轻患者的痛苦,提高临床疗效。

The clinical nursing in the treatment of the acute exacerbation of chronic obstructive pulmonary disease patients with respiratory failure by using the noninvasive ventilation

Objective： To observe the complications of acute exacerbation of chronic obstructive pulmonary disease （AE- COPD） patients by using the noninvasive ventilation treatment and summarize with the clinical nursing measures during the treatment. Methods： Collected the data of 102 patients who suffered from chronic obstructive pulmonary disease from May to October 2011 in the department of respiratory medicine in Yiwu central hospital,and analyzed the complications after using the non-invasive ventilation treatment. Results： Most of the patients appeared to have the related complications, there were gastrointestinal symptoms in 68 cases （66.7%）, fear in 42 cases （41.2%）, compression injury in 80 cases （78.4%）, dryness of eyes, mouth and nose in 56 cases （54.9%）, expectoration difficulties in 88 cases （86.2%）, aspiration pneumonia in 30 patients （29.4%）. These symptoms can be improved after strengthening the nurse care. Conclusion： There are many adverse reactions and complications by using the noninvasive ventilation in the treatment of acute exacerbation of chronic obstructive pulmonary disease patients. Through the actively nursing, we can relieve the patients＂ pain and improve the clinical curative effect.
