颅眶交界区显微解剖研究

摘    要:目的研究颅眶交界区域显微解剖。方法从不同的方向对26侧颅眶交界区进行显微镜下解剖研究,并留取图像资料。结果颅眶交界区的侧方暴露有利于眶-翼腭窝-颞下窝沟通肿瘤的切除,额底眶顶暴露适合于眶-蝶沟通肿瘤的切除,海绵窦和眼眶沟通肿瘤的暴露要兼顾额叶底面、中颅窝底前部和眶外侧壁暴露以颞侧上方入路为佳,内侧中线的暴露适合于鼻-眶沟通肿瘤的暴露。结论颅眶交界区与眼眶后部、海绵窦前部、蝶鞍区、鼻腔顶部筛窦、颞下窝和翼腭窝相关,不同特点肿瘤可以通过不同的方向和手术入路暴露。

Abstract:Objective To study the microsurgical anatomy of cranio-orbital region. Methods 26 cranio-orbital specimen dissected from four different directions under the microscopy and pictures collected at the same time. Results The lateral exposure of cranio-orbital junction is suit for tumors located in the junction of orbital cavity, pterygopalatine fossa and infratemporal fossa. The subfrontal transorbital approach is good for orbito-sphenoidal tumors. Fronto-temporal exposure include subfrontal area, middle cranial fossa and the lateral aspect of orbit is the best choice for occupying lesions in posterior part of orbit and the anterior part of cavernous sinus. Medial exposure is appropriate for naso-orbtal tumors. Conclusion The cranioorbital region is related to the posterior part of orbit, anterior part of cavernous sinus, sellae region, ethmoidal sinus and nasal cavity, infratemporal fossa and pterygopalatine fossa. Different kind of tumors may be approached from different direction.
