应用螺纹穿通导管治疗冠状动脉慢性完全闭塞病变

目的探讨螺纹穿通导管(Tornus)在冠状动脉慢性完全闭塞病变介入治疗中的应用价值。方法对我院收治的导丝能通过而最小径球囊导管不能通过的冠状动脉慢性完全闭塞病变5例的临床资料进行分析并复习相关文献。结果 5例均采用Tornus导管通过闭塞段血管,球囊导管成功扩张后置入合适支架,完成血管重建,手术时间1.0~3.0(1.9±0.7)h,造影剂用量150~300(240±42)ml。术中、术后及随访无并发症及死亡事件发生。结论 Tornus导管有助于治疗导丝能通过而球囊导管不能通过的冠状动脉慢性完全闭塞病变,提高介入治疗成功率,且操作简单,并发症少。

Penetration Tonus Catheters in Treatment of Chronic Total Occlusion in Coronary Arteries

Objective To explore the value of penetration tornus catheters （Tornus） in interventional therapy of chronic total occlusion （CTO） in coronary arteries. Methods Clinical data of 5 patients with CTO in coronary arteries, fol- lowing successful interventional therapy by Tornus catheters, while the smallest diameter Foleyg tube failed crossing the occlu- ded segments, was retrospectively analyzed, and related literature was also reviewed. Results Occluded segments were suc- cessfully crossed by Tornus catheters in the 5 patients, which enabled subsequent balloon dilatation, and then appropriate stents were placed in all the 5 patients, and the revascularization was carried out. The operation time was 1.0 -3.0 （1.9 -＋ 0. 7）h, and the amount of contrast media was 150 -300 （240 ~42）ml. There was no complication or death case before and after the operation and follow-up. Conclusion Tornus catheter may help treat patients with CTO in coronary arteries, whose occluded segments fail crossing by Foley＇s tubes, but succeed by Tornus catheters. It may increase the success rate of interven- tional therapy and the method is simple with few complications.
