摘    要:目的 为确定外周血有核细胞中LUNX基因、CK19基因和S100A4基因是检测非小细胞肺癌外周血微转移的重要指标提供实验依据.方法 术前抽取外周血,通过荧光定量PCR (PCR)检测其肺组织特异性蛋白(LUNX)、细胞角蛋白19(CK19)和钙结合蛋白A4(S100A4)的表达情况,并对其进行半定量分析.结果 肺癌患者外周血标本中LUNX mRNA、CK19 mRNA和S100A4mRNA的阳性表达率分别为66.7%、62.5%和56.3%,明显高于肺良性疾病患者和健康志愿者(P<0.05),这三种基因的表达与组织学类型、肿瘤位置有关,在腺癌中的阳性表达高于鳞癌,在周围型肺癌中的表达高于中央型肺癌.3种基因的表达与肿瘤的大小无明确关系.随着肿瘤分期的增高和淋巴结转移的发生,三种基因的表达有明显增高的趋势.结论 LUNX mRNA和CK19 mRNA、S100A4mRNA可作为检测肺癌微转移的分子标志物.

Abstract:Objective To explore the clinical significance of detection of lung- specific X protein (LUNX) gene,cytokeratinl9 gene and S100 calcium binding protein A4 gene on diagnosis of micro metastases in peripheral blood in patients with non - small - cell lung cancer. Methods The expression of LUNXmRNA,CK19mRNA and S100A4mRNA in patients'peripheral blood were detected and analyzed semi -quantitatively using fluorescence quantitative PCR (FQ -PCR). Results The rate of LUNX, CK19 and S100A4 posi- tive expression in patients" peripheral blood with lung cancer were 66.7% ,62.5% and 56.3% , respectively, which were significantly higher than those in patients with benign lung disease and healthy volunteers. The three genes'expression had no correlation with histolog- ical type and tumor location, but had increased tendency with neoplasm staging increasing and lymph node metastasis occurring. Conclusion LUNX, CK19 and S100A4 are appropriately targeted genes for the detection of micro metastasis in NSCLC.
