右美托咪定对全麻下经腹腔镜甲状腺手术患者应激反应的影响

目的：观察右美托咪定对全麻下经腹腔镜甲状腺手术患者应激反应的影响。方法：选择拟行全麻下经腹腔镜甲状腺手术患者40例,ASAⅠ或Ⅱ级,随机分为右美托咪定组（D组）和对照组（C组）。两组分别于麻醉诱导时在15 min内静脉泵入1μg/kg右美托咪定和等容量的生理盐水,并随后分别以0.5μg/（kg.h）持续静脉输注右美托咪定和等速生理盐水。检测麻醉诱导时（T0）、切皮时（T1）、切皮后30min（T2）和手术结束时（T4）去甲肾上腺素（NE）和皮质醇（Cor）的血浆浓度。结果：与T0比较,C组T2、T3时血浆的NE、Cor浓度显著升高（P〈0.05）;D组的血浆NE、Cor浓度变化无统计学差异。结论：全麻期间持续静脉输注右美托咪定可有效抑制经腹腔镜甲状腺手术患者的应激反应。

Effects of dexmedetomidine on stress response during laparoscopic thyroid surgery

Objective：To observe the effects of dexmedetomidine（Dex） on the influence of stress reaction of laparoscopic thy- roid surgery patients set to general anesthesia. Methods： Choose laparoscopic thyroid surgery patients under general anesthesia for 40 cases, ASA I or I1 level, randomly divided into dexmedetomidine group （group D） and control group （group C）. Two groups in anesthesia induced respectively in 15 min inside vein pump into 1 μg/kg dexmedetomidine and etc capacity of the physiological saline. Then continue to intravenous infusion 0.5μg/（ kg ·h） and uniform physiological saline. Detect the plasma concentration of norepinephrine （NE） and cortieosteroid （Cor） during anesthesia induction （T0）, before incision （T1）, incision after 30 min （T2） and at the end of the surgery （33）. Results： Comparison results and TO, the concentration of plasma NE and Cor significantly increased （P 〈 0.05） at T2, T3 in group C; Plasma NE, Cor concentration change of Group D was not statistically significant. Conclusions： A continuous intravenous infusion of dexmedetomidine during general anesthesia can effectively inhibit the laparoscopic thyroid surgery patients with stress reaction.
