医院优质护理示范病区护理人力配置研究

摘    要:目的通过对南通市6所三级医院优质护理示范病区护理工作量进行调查,确定各病区实际护理工作量,旨在建立优质护理示范病区人力资源配置模型,为政府制定"优质护理服务示范工程"新形势下的护理人力资源配置标准提供依据。方法采用现场观察法,对6所医院42个病区在班所有护士工作量进行现场观察,记录所需时间。结果 6所医院的实际病房床护比均低于各自的需配置床护比;心内科、神经内科、普外科、骨科、妇科、产科、儿科需配置床护比分别为1:0.56、1:0.67、1:0.79、1:0.56、1:0.47、1:0.70、1:0.72;A、B、C类病区平均护理时数分别为168.1、161.1、165.2 min/d,差异无统计学意义(P>0.05)。结论尽快改变护士缺编现象;由于各所医院制定的A、B、C类病区分类标准不统一,所以不能以示范病区类别配置护理人力,建议将病区平均护理时数作为配置护理人力的金标准。

Abstract:Objective To investigate the nursing workload in the demonstration ward of high quality nursing in 6 grade Ⅲ hospitals of Nantong and determine the actual nursing workload in order to build a model of nursing manpower allocation in the demonstration ward of high quality nursing and provide the foundation for formulating the standard of "demonstration project of high quality nursing".Methods Nursing workloads of all the nurses in the 42 wards of 6 hospitals were observed by the use of field observation method,and the required time was recorded.Results The In all of 6 hospitals,the actual bed-nurse ratio was lower than the desired bed-nurse ratio;the desired bed-nurse ratios in the 6 departments were 1:0.56(department of cardiology),1:0.67(department of neurology),1:0.79(department of general surgery),1:0.56(department of orthopedics),1:0.47(department of gynecology),1:0.70(department of obstetrics) and 1:0.72(department of pediatrics);3.the mean nursing hours in A,B and C wards were 168.1 min/d,161.1 min/d and 165.2 min/d,and there was no statistical difference(P>0.05)among the three wards.Conclusion Phenomenon of shortage of nursing establishment should be changed quickly.Because of the different standards of class A,B and C wards in different hospitals, the classification of demonstration?ward can not be a standard for nursing manpower allocation,and thus the mean nursing hour can be suggested as the important standard for nursing manpower allocation.
