中心静脉导管在腹腔热灌注中的应用

目的：观察中心静脉导管在腹腔热灌注治疗癌性腹水的疗效及不良反应对患者生活质量的影响。方法：将48例癌性腹水患者随机分为应用中心静脉导管腹腔热灌注化疗组（热化疗组）24例和腹腔常温用腹腔穿刺包灌注化疗组（化疗组）24例。热化疗组注入已预热到45℃的生理盐水＋顺铂;化疗组注入常温下的生理盐水＋顺铂;每周1次,共治疗2～3次。观察治疗后腹水控制情况及生活质量。对比两组的治疗效果。结果：两组的腹水控制率、生活质量改善率分别为75%（18/24）、82.5%（19/24）,化疗组50%（12/24）、45.08%（11/24）;两组间有显著差异。而两组间的化疗毒副作用无明显差异。结论：应用中心静脉导管腹腔热灌注化疗治疗癌性腹水优于常温常规腹腔化疗。

Clinical Application of Central Venous Catheters in Intraperitoneal Hyperthermic

Objective： To observe the clinical effects of the treatment for rhalignant ascite by central venous catheters in intraperitoneal hyperthermic, and adverse reactions on the life quality of patients. Methods： A total of 48 patients ere randomly divided into thermal perfusion chemotherapy group in treatment normal saline preheated to 45℃ with combined with cisplatin, control group with normal saline, once per week , for 2 - 3 weeks, the control of ascites and the life quality were observed. Results ： the control rates of ascites and quality of life improvement rate were 75% and 82.5% separately, and distinguished markedly compared with control group （50% ,45.08% ）, but no diferences were found in the efficiency between them. Conclusions： the effect in treatment for malignant ascite by central venous catheters in intraperitoneal hyperthermic was superior to routine treatment.
