Ⅰ期子宫内膜癌167例临床分析

目的:探讨Ⅰ期子宫内膜癌生物学行为和相关因素。方法:对该院1997年1月～2006年12月收治的167例Ⅰ期子宫内膜癌患者临床资料进行回顾性分析。结果:临床Ⅰ期盆腔淋巴结转移率为11.3%;术前分段诊刮加B超检查,对判断浸润深度有一定价值,但与术后病理检查相比,差别有显著性意义(P<0.01)。经单因素分析:浸润深度、病理分级和组织学类型均与盆腔淋巴结转移有关(P<0.05)。结论:分段诊刮加B超检查对判断浸润深度有一定价值,但需提高精确度。Ⅰ期子宫内膜癌盆腔淋巴结转移与子宫肌层浸润深度、组织学类型和病理分级密切相关。临床Ⅰ期低危组可仅行全子宫双附件切除,不必盆腔淋巴结清扫。

A clinical analysis of 167 cases of stage Ⅰ endometrial carcinoma

Objective:To explore the biological relative factors of endometrial carcinoma(stageⅠ).Methods:167 patients with endometrial carcinoma(stageⅠ) accepted primary surgical treatment from January 1997 to December 2001 were analyzed retrospectively.Results:In stageⅠof endometrial carcinoma,the metastasis rate of pelvic lymph nodes was 11.3%.It was certain value to perform fractional curettage and B-ultrasongraphy in judging the depth of myometrial invasions,but it had closely relation comparing with post-operative pathologic examination(P<0.01).The depth of myometrial invasions,pathological grades and histological types were related to the extent of the pelvic lymph nodes metastasis by univariate analysis(P<0.05).Conclusion:It is certain value to perform fractional curettage and B-urtrasongraphy in judging the depth of myometrial invasion,which needs elevation of its accuracy.Depth of myometrial invasions,pathological grades and histological types are closely related to the metastasis of pelvic lymph nodes.It need only performing total hysterectomy puls bilateral salpingo-oophorectomy and unnecessary to perform retroperitioneal lymph nodes dissection for patients with cases of low-risk stage.
