某院甲状腺手术患者围手术期抗菌药物应用调查分析

目的了解某院甲状腺手术患者围手术期预防性应用抗菌药物的情况。方法对2006年6月-2008年6月期间478例甲状腺手术患者围手术期所应用抗菌药物种类、用药时机、疗程、联合应用及更换情况进行统计分析。结果478例手术患者均在围手术期使用抗菌药物，使用率100％，其中Ⅲ代头孢菌素使用率为58．37％，术前30min-2h给药18例占3．72％，术后用药451例占94．35％，平均疗程5．59d。表明该院在围手术期预防性应用抗菌药物存在用药选择不合理、时机不当、重视术后用药及用药时间过长等普遍问题。结论应加强围手术期抗菌药物合理使用的管理和监督，促进临床医生合理用药。

Perioperative prophylatic use of antibacterials inpatients undergoing goiter surgery

Objective To investigate the preoperative use of prophylactic use of antibacterials in patients undergoing goiter surgery. Methods The perioperative prophylactic use of antibacterals in 478 patients of a hospital undergoing goiter surgery was analyzed statistically in respect of drug variety, medication time, treatment period,drug combination etc. Results 478 cases （100%） received peripretative prophylactic antibacterials. The usage rate of third generation cephalosporins was 58.37%. 30 minites-2 hours before operation accounted for3.72% （18 cases） . 451 cases（94. 35% ） received antibacterials after operation. The mean antimicrobial course was 5.59 days. Many problems about preoperative prophylactic use of antibacterials exists in the hospital, for example, the irrational choice of drug, inappropriate medication time, emphasizing the postoperative use, the prolonged use of antibacterials etc. Conclution To accelerate rational use of drug, it is urgent to intensify management and intendance about rational perioperative use of antibacterials.
