宫颈癌阴式广泛性子宫切除术后下尿路功能的研究

目的通过对宫颈癌患者接受阴式广泛全子宫切除手术前后的下尿路功能进行检查,探讨尿动力力学检查的价值。方法 50例ⅠA2～ⅡA期宫颈癌患者在宫颈癌根治术前及术后接受尿动力学检检查,包括充盈性膀胱测压和压力流率同步测定。结果阴式广泛全子宫切除患者术前、术后的尿动力学检查结果比较有明显变化。术后平均尿流率、最大尿流率、排尿量、膀胱顺应性、最大尿流率时逼尿肌压明显下降。而膀胱初始尿意容量、最大膀胱容量、残余尿量明显高于术前。术后平均尿流时间比术前延长。术后3个月下尿路功能明显改善。结论宫颈癌根治术后患者的下尿路功能改变明显,尿动力学检查对术后下尿路功能障碍的诊断及指导治疗有重要义。

Effect of radical hysterectomy on lower urinary tract function

Objective To study the value of urinary dynamic in cervical cancer patients who underwent vaginal radical hysterectomy.Methods 50 patients with stage Ⅰ A2~Ⅱ A cervical cancer received urinary dynamic check pre-and post-radical hysterectomy surgery.Results The mean postoperative urinary flow rate, maximum urinary flow rate, urinary volume, bladder compliance,detrusor pressure at maximum urinary flow rate was remarkably decreased. Bladder initial urine capacity, maximum bladder capacity, residual urine volume were higher than that before surgery. The mean postoperative urinary flow time was prolonged.lower urinary tract function improved obviously three months after operation.Conclusion Lower urinary tract function changed significantly after vaginal radical hysterectomy, urodynamic examination has value on the diagnosis of lower urinary tract dysfunction.
