经化疗泵植入术行肝动脉介入治疗晚期原发性肝癌

目的：评价对无法切除的原发性肝癌患者实施经化疗泵植入术行肝动脉介入治疗的疗效。方法；28例晚期原发性肝癌患者均行化疗泵植入术。术中及术后经泵注入肝动脉化疗药物，其中20例行碘油栓塞。术后观察病情变化，并定期复查AFP定量、B超或CT检查以评价疗效。结果：所有患者临床上均有不同程度的症状减轻，81%患者术后2周-1月复查AFP明显下降，64%患者肿瘤缩小明显，术后半年、1年、2年生存率分别为82%、64%、43%，尤以巨块型行栓塞者疗效好。结论：化疗泵植入术与常规股动脉穿刺插管术比较具有安全可靠、选择性强、给药简便及并发症少等优点，对晚期原发性肝癌的治疗是可行的，疗效是可靠的。

ARTERIAL CHEMOTHERAPY FOR ADVANCED HEPATOCELLULAR CARCINOMA BY IMPLANTABLE PUMP

Objective To evaluate the effect of arterial chemotherapy for advanced hepatocellular carcinoma by implantable pump.Methods Between Nov.1995 and June 1999,twenty eight patients with unresectable hepatocellular carcinoma received transcatheter hepatic artery chemotherapy by implantable pump system.The anti-tumor agents were 5-FU,MMC and ADR.Twenty cases were given lipiodol fluid chemotherapeutic embolization.The data of AFP,B mode ultrasonography (or CT) and clinical features were studied after operation.Results Clinical symptoms alleviated to different degrees in all patients.The AFP level declined in 80% cases and tumor size reduced in 64% cases.The survival rate in half year,one year and two years after operation were 82%,64% and 43%,respectively.Conclusion This implantable infusion pump is safe,simple and selective compared with conventional arterial chemotherapy for the treatment of unresectable liver malignancies.It appears to be a practical means with long-term delivering for outpatients.
