摘    要:目的探讨急诊条件下术中实时超声导航进行微侵袭脑内血肿清除术的方法和临床效果。方法本组急诊手术病例36例,诊断包括自发性和创伤性脑内血肿,所有病例在清除血肿前进行术中实时超声导航定位,手术在显微镜下行血肿清除。结果36例术后复查结果提示血肿清除彻底,无残留血肿。GOS预后评分,良好29例,中残4例,重残3例,死亡2例。结论术中实时超声在急诊脑内血肿清除术中应用能够更好的定位血肿,为选择合理的皮层切口提供了良好的帮助。

Abstract:Objective To explore the application of intraoperative ultrasound guided micro-neurosurgery in emergent operation for intracranial hematoma evacuation.Methods Emergent operation were performed in 36 patients with traumatic and spontaneous intracranial hematoma.Ultrasound was used for locating hematoma depth in hemisphere just before cortex incision.Operation was completed by microscope.Results CT images demonstrated that hematoma were fully evacuated in 34 cases.As Glasgow Outcome Scale,good recovery in 29 patients,moderately disabled in 4 patients,severely disabled in 3 patients and dead in 2.Conclusions Intraoperative real-time ultrasound is helpful to locate hematoma depth in hemisphere when emergent operation were performed,and to decrease the injury of operation.
